Alternative Solutions is a ‘hands on’ NHS social prescribing programme supporting individuals aged 18 years and over who live in Cheshire and are registered at GP practices within the Primary Care Networks of:
Nantwich and Rural Areas, Crewe (Eaglebridge), Crewe Grosvenor and Hungerford Rope Green
The programme supports individuals with a wide range of social, emotional or practical needs to take greater control of their own health and wellbeing. Individuals can be referred to the Alternative Solutions programme via their GP practice.
For GP practice staff, you can refer your patients via social prescribing found on EMIS.

On referral to Alternative Solutions, individuals are assigned an individual link worker who provides tailored support, working as a partner towards improved health and wellbeing.
This can be on the phone, face to face, or within the community at GP surgeries.

Be Well
Over the years, Pathways CIC has developed prototypes and delivered specialist ‘In-Work’ Support services that are designed to support our clients, who are in employment, but who may be struggling to stay in work due to their health, a health condition or a disability. Our services are designed to provide solutions for clients for common sickness absence, including mental health and physical health. We have a team of specialists who will design a bespoke intervention programme to help clients stay in, or return to, work, based on their needs.
Pathways CIC is a partner to Be Well Manchester, which works in partnership with the NHS, Manchester City Council and other stakeholders to deliver health and wellbeing innovation.
