Who is eligible to access our services?

For healthcare professionals in:

  • South Cheshire,
  • Vale Royal,
  • Congleton and Holmes Chapel,
  • Chelford,
  • Handforth,
  • Aldersley Edge and Wimslow

You can refer individuals aged 18 years and over who require support and guidance to address social, emotional or practical needs to take greater control of their own health and wellbeing, including workplace wellbeing via their GP surgery, citing that the person would benefit from social prescribing.

We provide a ‘hands on’ social prescribing programme designed around the individual to address the consequences of non-medical issues on individual lives and wellbeing.

For GP practice staff, you can refer your patients via social prescribing found on EMIS.

We also deliver multiple services across Greater Manchester which offers support to those in work who are struggling with their health, wellbeing and social issues or those who are currently off work sick.

For Healthcare professionals in:

  • South Manchester,
  • Central Manchester,
  • North Manchester

You can refer your patients in to the Be Well service via EMIS alternatively you can email bewell.mcr@nhs.net.

For Healthcare professionals across Greater Manchester with the exception of Manchester you can refer your patients in to the Working Well Early Help service by searching WWEH on EMIS/Vision.

Working in tandem with healthcare professionals

Working in tandem with healthcare professionals, medical staff and patients, we create, manage and maintain a patient’s healthcare plan. We also use asset-based community development to support the sustainable development of communities based on their strengths and potentials. Pathways CIC also partner with organisations offering the Be Well, Working Well Early Help and Working Well Work and Health programmes.
