Pathways 50+ service provides support with:

Managing health conditions Personal finances Improving skills Increasing confidence, motivation, positivity and hope Finding work, volunteering and training

How does it work?

Once a client has been accepted into our programme, our team will conduct a telephone-based assessment to pinpoint the unique needs of the client. Together, we will establish SMART goals with a primary focus on creating positive changes in their overall health and well-being. All of our support is personalised and tailored to cater to each client’s specific requirements with the aim of dismantling barriers and empowering our clients to access fresh opportunities such as training, volunteering, and employment.

Following the initial assessment, clients gain access to up to 19 hours of support spanning over a flexible 3-6 month period. This support can be delivered through personalised 1-2-1 interventions, whether it be through telephone consultations or in-person face-to-face meetings. We also offer a diverse range of workshops and peer-to-peer support groups, allowing clients the freedom to engage as they see fit.
