Be Well

Pathways CIC works in partnership with Be Well Manchester, offering support to clients who are employed and off work sick or at risk of going off work sick, to help them return to or stay in work by providing holistic early intervention.
The employed element of the service is telephone based and clients will be supported by their allocated Link worker (or Work and Health Coach), their role is to understand the client’s current circumstances, challenges with work, health and wellbeing, condition management and its effect on their employment/lifestyle amongst other factors.
We have a team of specialists at hand who work collaboratively to ensure fast access to Physio/Chiropractic treating, MH Coaching/Counselling and HR. You can refer your patients into the Be Well service via EMIS alternatively you can email bewell.mcr@nhs.net.

One You Cheshire East
Pathways CIC works in partnership with Everybody Health and Leisure to deliver the Community Link Worker element of the One You programme across Cheshire East. Our Link Workers are based within local communities, raising awareness of One You among residents and professionals to drive referrals and improve long-term health outcomes. By building trusted relationships and engaging harder to reach groups, they help people understand the support available and access the help that best meets their needs.

Economic Activity Trailblazer (ETB)
The Economic Activity Trailblazer (ETB) programme is a Manchester City Council initiative designed to support residents who are economically inactive to improve their health, wellbeing and progress towards employment. The programme brings together a range of partners across the city to deliver targeted, community-based support tailored to specific populations. Through this work, Pathways CIC contributes by providing specialist mental health support, helping individuals overcome barriers, build confidence and move closer to sustainable employment, while also supporting the wider Work and Health agenda across Manchester.

Alternative Solutions
Alternative Solutions delivers a gold standard of expertise through our dedicated Social Prescribing Link Workers, who are integral to the personalised care journey of patients accessing their local GP surgery or Primary Care services. By focusing on the principle of “What Matters to Me” rather than “What is the Matter with Me,” we empower individuals to address health inequalities and take proactive steps toward better health and wellbeing.
Aligned with the NHSE Workforce Plan, our approach provides GPs with the time to focus on medical needs while our Social Prescribing team addresses the broader social determinants of health. From tackling loneliness and isolation to supporting housing, debt, and employment challenges, we work to enhance lives by providing compassionate, tailored support for every individual.
