Pathways Community Interest Company (CIC), since inception in 2006, has a long history of working with communities from across Cheshire to support their health and wellbeing. Our work over the years, through both our community development programmes and social prescribing programmes, through commissioned programmes across both healthcare and Local Authorities, has enabled us to develop relationships with residents and local services that have largely been designed to support individuals’ health, how they look after themselves and accessibility.
We are recruiting for a Community Link Worker, with occasional weekend and early evening event attendance required.
This position involves actively promoting and gaining referrals for the One-You Cheshire East programmes. Supporting local communities to start and maintain a healthier lifestyle. As a Community Link Worker you will play a pivotal role in supporting residents to access the One You Cheshire East service, particularly those facing health inequalities such as individuals from deprived areas or those typically difficult to engage with.
